Has 3 holes, only 2 screws in package, should have 3 . The only left hand thumb rest I know of.
Response from chuck- The three holes are not meant to be all used, only two. The three holes are to allow for more adjustability as to where you locate it.

This one sits on my Tanfoglio Limited which had pre-drilled holes for mounting. Blue Loctite is a must for both mounting holes and the adjustment screw. Once set up, provides a solid, large and wide platform for support thumb.
